CPS has been providing Support Coordination Services since 2013 and was one of the first agencies in New Jersey authorized to self-approve budgetary plans. We are a state-approved provider of Support Coordination Services in Essex, Union & Somerset counties and in the surrounding areas of service. Our many years of experience allow for quick plan changes and approvals.
Support Coordinators assist individuals and their families to access and receive services in their local and surrounding communities. Plans and services are individualized to meet the hopes, wants and dreams of each person served. Our Person-Centered approach leads to positive, self-directed outcomes. Our staff is readily available and accessible. We provide 24-hour emergency support!
Staff will:
- Build strong relationships with individuals and supporters.
- Assist individuals in identifying and charting a life course.
- Connect individuals with resources that meet their preferences and abilities; helping them progress toward their hopes and dreams.
- Construct a Person-Centered Plan that grows and changes with the individual’s preferences and needs.
